The formal merger of Blakeney Surgery and Coleford Medical Practice will take place on 1st August 2025 to become Dean Medical Practice. This will require an update to our clinical system, which will take place on 4th August 2025. During this time patients will unfortunately experience reduced services.
Please bear with us while the updates to the system are being implemented. We would like to thank all our patients for their consideration to our staff during this change.
Disruption during system merger
During the system merger, the practice has limited access to systems, therefore we request that you only contact the practice if your condition is urgent during this period.
Think Pharmacy First
If you have a common condition such as a cold, cough, sore throat please visit your pharmacist for advice.
Pharmacists can now provide treatment or some prescription medicine, if needed, for seven common conditions, without having to see a GP. Each condition is subject to age eligibility when providing prescription medicine:
Sinusitis, Sore throat, Earache, Infected insect bite, Impetigo (a bacterial skin infection), Shingles, Uncomplicated urinary tract infections in women.

What about my Repeat Medication?
We will be unable to process any routine repeats of medication between the 1st and 4th August, so please ensure you order any medication you may require in plenty of time. We will be issuing supplies in advance if appropriate to minimise any disruption.
We will be able to send prescriptions electronically to your pharmacy from 5th August 25.
Can I still use my online service for appointments and repeat prescriptions?
No, online services will be switched off during from 30/07/25 and will be unavailable until 8/8/25.
Please use the online forms on the Practice website to order repeat prescriptions until online services can be resumed. https://colefordmedicalpractice.nhs.uk/services/managing-your-health-online/online-forms/repeat-prescription-request/
If you have an Apple device or Smartphone, you can download the NHS App. You will need new registration details, so please ensure the practice has your up-to-date mobile number and email address. If you currently use the NHS App with an NHS login, you may need to log out or re-install the App so that it links with the new system.
If you are a new user please create an NHS Login https://www.nhs.uk/nhs-services/online-services/nhs-login/ or complete the online registration form and provide photo ID and something with your address on.
If you do not have these, please talk to us and we will help you sign up.
Can I register at the practice during this transition period?
We can accept registration forms during this period; however, the registration cannot be processed between 30/07/25 and 05/08/25. Registration forms will be processed as soon as possible after the merger.
